Input in table format

In the Demo survey that comes with Win Survey, for question #5 Loadgingfacilities, it shows input in a table format, ie. 6 x 5. How can I create this type of question? Thanks.....

Hello,

this is question type "Choose one per topic". Question tab : Add your scale via the scale options on the right side of the window (in example this is from "Excellent" to "Poor"). Answers tab : Add your answers here (in example this is "Restaurant" , "Bar" and so on).
